It has come to the attention of the OCMA Board of Trustees that members of OCMA may benefit from an educational presentation concerning pensions and 401 (k) plans. To that end, we have invited representatives of Everhart Financial Group, Inc. in Dublin, Ohio to address our membership at the January 2007 meeting scheduled for Tuesday, January 16, 2007. To assist them in preparing their presentation, they have provided us with the attached survey form. The form was included in the membership packet at the October HR Day meeting and a significant number of surveys were completed and returned. However, since many OCMA company representatives were not present at the October meeting, the Board has instructed me to send it to you and to ask you to complete the survey and return it to the OCMA office.

Please give due consideration to completing the attached survey form and returning it to the OCMA office by fax, (614) 876-3615, mail, or e-mail. If you are interested in maintaining strict confidentiality of your survey information, I would suggest that you send it by mail in a non-stationery envelope to the OCMA office at 2969 Scioto Place, Columbus, OH 43221.
